As an RN Liver Pre Transplant Coordinator with Clinical Transplant Services, you are integral to our team. You will help guide our patients through the transplant process to get on the liver transplant list and maintain their status. Providing initial and ongoing patient and family education is crucial. The RN functions as a liaison between the patients and staff members of the transplant team (physicians, social worker, financial counselor, psychiatrists, etc.) and has the primary responsibility for providing direct and indirect professional nursing care for patients. The RN is responsible for screening and educating potential recipients, direction of the evaluation process, facilitation of committee review, direction of additional testing and update testing, and management of the waiting list. The RN provides initial and on-going patient/family education in a variety of settings. The RN is responsible for collection and evaluation of patient laboratory testing, diagnostic procedures, consults, and regulatory requirements. The RN provides post-transplant cross coverage for essential functions. The RN assesses patient and family needs, develops a plan of care, implements and evaluates the care delivered, working closely with all members of the inter-disciplinary team. The RN is expected to practice in accordance with professional standards, core values, and with a commitment to service excellence.
