Department Overview

Family Medicine at Gabriel Park is a community clinic located in SW Portland. We serve our patients in varying stages of life and throughout their lives. Our services include general family medicine, transgender care, maternity care (including deliveries), pediatric care, behavioral health, sports medicine, travel immunizations, acupuncture, a retail pharmacy, x-ray, minor procedures, and health maintenance with focused preventive care. We are a high volume clinic that sees up to 250 patients and receives 400-600 phone calls daily. We have 39 providers plus 60 staff members. Our clinic guides itself though our vibe: Compassionate, Committed, and Collaborative.

The OHSU Clinical registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The Clinical RN’s care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The Clinical RN demonstrates the professional role obligations of scientist, leader, practitioner, and knowledge transferor [O’Rourke Model of the Professional Role™]. Description of work:

Responsible for the delivery of appropriate, timely and beneficial care for patients which promotes quality and cost-effective health care outcomes working within established standards for case management practice, and Advance Access principles. Provides standard nursing care for patients throughout the age continuum. Responsible for active management of patients. Maintains compliance with applicable laws such as client confidentiality, Americans with Disabilities Act, Workers’ Compensation, abuse reporting, principles of consent, and advanced medical directives. Advocates for the client/family at the service-delivery level and at the policy-making level fostering the client/family’s decision-making, independence, and growth and development. Integrates factors related to quality, safety, efficiency, and cost effectiveness in planning, delivering, monitoring and evaluating client care promoting the most effective and efficient use of human and financial resources. Functions as the liaison between referral providers and GP providers. Participates in clinic, department, OHSU campus, teams and committees. Assist in the implementation of nursing system that supports the mission of the American Academy of Ambulatory Care Nursing. (AAACN).

 

Clinical Skills and Duties:

  • Participate in Risk Stratification for team patient population
  • Participate in the development of Care Management
  • Perform hospital follow up calls.
  • Follows standing orders/protocols in assisting with patient care.
  • Cross coverage of all clinical teams as needed.
  • Provides team leadership.
  • Uses EMR to assist with patient care.
  • Clinical skills to include but are not limited to; INR testing, IV placement/infusion, bladder catheterization, suture removal, immunizations/injections.
  • Manages nurse visit schedule which includes but is not limited to: B/P checks, wound care, suture removal, education/teaching, INR testing, prenatal intakes.
  • Responsible for handling urgent/emergent calls and walk-ins per clinic approved advice protocols.
  • Reviews the visit schedule to anticipate healthcare needs of patient panel.
  • Actively coordinates care for patient care as needed.

Required Qualifications

  • All candidates must have 6 months of clinic experience within the last 2 years.
  • External applicants: Baccalaureate of Science in Nursing Degree from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA)
  • Internal ONA applicants: Associate's Degree in Nursing
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA) or Military Training Network branch of AHA (upload documentation with online application)
  • BLS must not expire during orientation or probation period, and must have the same expiration date (upload documentation with online application)
  • Must be able to perform the essential functions of the position with or without accommodation
  • Advanced Cardiac Life Support (ACLS) certification is required.

Additional Details

Expected abilities of the RN:
 

Ability to perform duties using theory-based nursing practice model, evidenced-based practices, and shared governance in a multidisciplinary team model. Ability to participate in a multidisciplinary clinical team. Knowledge of clinical quality improvement models and process. Knowledge of scope of patient care services provided by other clinical service lines and the referral process. Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care. Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., JCAHO. Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions. Knowledge of clinic practices and ability to act as a resource for clinic personnel and assist the providers as necessary. Proficiency in the use of appropriate EMR, computer based software and office equipment. Ability to maintain quality safety and infection control standards. RN must possess the ability to apply the operational principles of the GPFM care model. Knowledge and ability to apply primary care best practices. Ability to mentor and orient new staff members.

 

Accountability:

Provides accurate and timely documentation of care of patients in the patient record - EMR. Assure proper transfer of patient care and coverage of duties when leaving the clinic. Initiate appropriate interactions with patient’s, providers and other staff regarding any relevant patient care or operational issues.
 

Policies and Procedures:

Implement agreed upon changes in function, policies and procedures. Establish appropriate patient/clinic flow priorities and follow through to completion. Participate in the formation, development, implementation, and evaluation of health care plans for patients. Other duties/projects which are assigned by supervisor and/or Lead Nurse are to be followed through to completion.
 

Meetings:

Attend and participate in relevant departmental, clinic, nursing, and patient care meetings. Assist and actively participate in the ongoing improvement of the clinic.
