Function/Duties of Position

The OHSU Digestive Health Center (DHC) is a multi-disciplinary center offering services in the ambulatory setting for specialty surgical services, gastroenterology, hepatology, bariatrics and nutrition.  The programs work in a close team that includes MD, RN, MA, and schedulers that work together to provide excellent quality care for complex patients. The patient experience with the program begins with their initial appointment and can include clinic visits, procedures, surgery, and follow-up care.

 

The Resource RN Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the teams in the ambulatory setting.  The nurse will perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews.  The nurse will work alongside a team of RN care coordinators within the DHC.

The qualifications for the position are the minimum requirements to be eligible for an interview. The level of required experience and expertise for each posted position is influenced by the current amount of knowledge and skills of the other nurses in the unit/departments.

 

*Current, unencumbered Oregon RN license

*Current BLS, required at time of hire

*Associate or Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA)

*BSN preferred

*Associate Degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire.

 

At least 1-2 years of nursing experience inpatient with a complex medical focus, or in ambulatory or community based setting working with chronically ill or medically complex patients.

  • Must be able to deal with competing needs and priorities
  • Requires ability to triage and assess complex patient symptoms/condition
  • Requires excellent time management skills
  • Heavy phone use and ability to use technology essential to position's success (EPIC)
  • Must be able to work independently
  • Will involve long periods of sitting throughout the day
  • The work environment carries risk of exposure to human tissue/fluids
  • Ability to use the senses of seeing (including color vision ability), hearing, touch, and smell to make correct judgments regarding patient conditions and safely administer care
  • Significant use of computers, electronic health record, copiers, multi-line phones, and fax
  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

Experience in the field of surgery / GI medicine in an Ambulatory clinical setting is preferred
