Department Overview

Columbia Memorial Hospital (CMH) located in Astoria, Oregon is a full service, 25-bed, critical access, not-for-profit, Planetree-Certified hospital where person-centered care is a priority. CMH has proudly served the healthcare needs of our community for over a century. Combined, the hospital and clinics employ over 700 employees which includes a diverse professional medical staff of more than 50 physicians and advance practice professionals including surgical, non-surgical and primary care specialties.

 

This position will work at the CMH-OHSU Knight Cancer Collaborative, located in Astoria, Oregon.

Function/Duties of Position

We are seeking a Family Nurse Practitioner to serve the efforts of OHSU and CMH in improving the health of all Oregonians. This position will serve as an OHSU Clinical Associate providing clinical care within the CMH-OHSU Knight Cancer Collaborative at Columbia Memorial Hospital (CMH) located in Astoria, Oregon. OHSU Clinical Associates are fully embedded physician and advanced practice provider members of the OHSU Practice Plan (OPP) who deliver care at OHSU-affiliated settings and devote 100% of their time to patient care.

 

Key responsibilities: 

  • Clinical:
    • Provider outpatient services at one (1) CMH Clinic: CMH-OHSU Knight Cancer Collaborative.
    • Clinic Schedule: 36 hours of patient care will be provided at the CMH-OHSU Knight Caner Collaborative; ten (10) hour clinic days.
    • Inpatient Rounding on medical oncology patients. Rounding responsibilities are shared among the CMH-OHSU Medical Oncology providers; inpatient responsibilities should not exceed five (5) percent of patient care hours.
    • Call schedule: Up to ten (10) days of call per month.
    • Responsible for clinical coordination of Palliative Care and the Survivorship Program. 
  • Administration:
    • Administrative and other duties may be assigned based on department needs and your areas of expertise and interest.
    • Four (4) hours of administrative time are allotted each week.

Required Qualifications

  • Master’s of Nursing
  • Oregon RN license
  • AANP Board Certification
  • Demonstrated ability to work with a team of health care providers and collaborate to achieve patient care goals.
  • Experience working in an interdisciplinary setting.
  • Communication skills and the ability to collaborate with other clinicians in the work unit.
