The Patient Navigator provides clinic-based case management services to adolescent patients enrolled in OHSU (Oregon Health & Sciences University) Healthshare cared for by DCH (Doernbecher Children’s Hospital) General Pediatrics and Adolescent Health. The Patient Navigator builds strong relationships with adolescent patients in order to help them stay engaged in their medical care, adhering to recommended immunizations, well-child checks, and appropriate age screenings. The Patient Navigator is committed to removing barriers to care by identifying critical resources for patients, helping them navigate through the healthcare system, transitioning care, promoting well-being and patient health. They work closely with the care team including doctors, nurses, behavioral health team, and other clinic staff to support positive health outcomes.


The Patient Navigator is responsible for analyzing IDS metrics and developing action plans to meets goals and initiatives. The Patient Navigator is responsible for using analytic tools to interpret data, identify patterns/trends and inform decision making and effectively communicate findings and next steps to appropriate stakeholders.


The Patient Navigator position requires medical training (Medical Assistant or Licensed Practical Nurse). To meet IDS metrics and patient care, this position would be responsible for administering immunizations, out of book as necessary to meet patient care needs.


This is grant funded position with limited duration from May 2024 through October 2025.

Function/Duties of Position

  • Track patient attendance at medical appointments and initiate outreach to patients over the phone, by letter, or My Chart to assist in setting up primary care appointments at DCH General Pediatric Primary Care Clinics or Adolescent Health. Yes
  • Work with patients, providers, and interdisciplinary team as needed to set goals for patients care
  • Review patients' chart for overdue well-child appointments, screenings, and/or immunizations.
  • Identify resources for clients to overcome barriers to care, such as transportations, housing, food insecurities, etc. in collaboration with social workers to meet patients' needs
  • Monitor and analyze IDS metrics related to well-child checks, immunizations, and age-appropriate screenings
  • Maintain proper documentation of client encounters in the EMR
  • Assist patients in transitioning of care to family or internal medicine
  • Maintain regular communication with the patient’s provider through written documentation.

Required Qualifications

  • High School Diploma

  • Experience with public health, youth advocacy, and/or adolescent patients in a primary care setting

  • Certification as Traditional Healthcare Worker (THW) required. Traditional Health Worker types include Doula, Peer Support Specialist (PSS), Peer Wellness Specialist (PWS), Personal Health Navigator (PHN) and Community Health Worker (CHW). Type preference may be determined by service need within 3 months of hire.

  • Community Health Care Worker required within 3 months of hire

  • Compliance with Code of Conduct, Respect in the workplace and Applicable policies, procedures and agreements related to position, department or OHSU as a whole

  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

  • 1-year experience in a clinical setting, including proficient knowledge of adolescent immunization, well-child checks, screenings, and community resources.
  • Multilingual

Pay Range: $24.62 - $33.28 per hour

  • This position will travel between Bethany Village, Campus Peds and Adolescent Health. Days: Monday-Saturday with variable hours.
