Department Overview

The Nurse Navigator will work collaboratively with the orthopedic total joint faculty experts and their interdisciplinary teams. The nurse navigator coordinates the care for the patient prior to the patient’s first consult visit at OHSU.  This position will work with total joint team to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient’s first visit and, through the treatment of their orthopedic condition.

 

A Nurse Navigator (NN) at OHSU is a professional RN with orthopedic-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.  The Nurse Navigator guides patients and their families through their joint replacement experience, implementing an integrated process of care to facilitate patients' intervention, and recovery. The NN interfaces with research staff, and works to identify patients who may be suitable candidates for clinical trials.

Function/Duties of Position

Hiring Bonus up to $10,000 DOE and Relocation Package Available. Must begin work by July 1, 2024 to qualify.  

 $49.16-$78.18 per hour, based on RN Degree and level of experience.

Conducts a patient assessment interview to identify patient’s understanding of  surgery, barriers to care, social determinants of health, preferred learning style, family support, and any current symptoms.  This position plays a key role on the interdisciplinary team as the single point of contact to coordinate care in the pre, intra and post-operative care and physical therapy leading up to and following a joint replacement performed at OHSU and partner hospitals.

The NN tracks data on each referral in order to capture information on the population served and to follow metrics for volume, access, referral sources, work up requirements, subsequent needs after consult.

The NN may meets with patients in clinic  or on the inpatient unit as determined by the specialty team.

Provides pre- and post-op education, educational resources and support to patients and their families. The NN acts as a resource to all members of the health care team related to provision of care, enhancing assessment skills of others, establishment of appropriate care standards, and determination of realistic goals for optimal patient outcome.

The NN ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period.  The NN works with the care coordination team and inpatient team on the admission and discharge plan. The NN will work with the orthopedic total joint APPs on triage of patient specific issues and concerns pre and post surgery.

Accountable for the key performance measures of the joint replacement patient and works with the Quality team to identify and track key performance measures, including but not limited to: Patient satisfaction, physician satisfaction, LOS, variable cost/case, discharge disposition, and orthopedic specific outcome tools as necessary.

Manages patient engagement platform and ensures completion of patient reported outcomes.

Required Qualifications

External Applicants: Baccalaureate of Science in Nursing awarded from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) by 30 days before the start date.

 

Requires three years of nursing experience including a minimum of two years orthopedic nursing experience.

 

Current, unencumbered Oregon RN license by 1 week before start date.

 

Current BLS certificationby the American Heart Association.

Preferred Qualifications

Current OHSU employees: BSN preferred.

 

Demonstrated ability to take a leading role in developing a new position to include:

  • Developing systems to track trends in patient barriers to care.
  • Collaborating with the team to create customized workflows to improve patient outcomes.

Additional Details

Shift 8 hrs M-F eigh hour shifts to include the 0800. Negotiable in alignmnent with needs of the speciaty team’s needs. Intermittent exposure to usual hospital environmental conditions including possible exposure to blood and body fluids for which staff is provided education and protective equipment to minimize risk. This position may be eligible for partial remote work after orientation and as deemed feasible by the rest of the interdisciplinary team. On campus work is required a minimum of one full shift per week.
