The Wellness Promotion and Outreach Manager is responsible for the day-to-day oversight of the Wellness and Outreach Program to ensure effective and efficient program performance and delivery of expected wellness promotion, education and outreach services. The Wellness Promotion and Outreach Manager is operationally responsible, reporting to the Care Integration and Coordination Program director, for oversight of the Care Integration Specialist Team. As the Manager, this position will facilitate the work of the program in supporting Members and Providers in addressing social determinant barriers in improving individual and community health and in identifying new systems to improve the delivery of care from a culturally-sensitive and linguistically appropriate approach. The Manager is primarily responsible for employee recruitment and performance to include, interviewing, on-boarding, coaching, counseling, disciplinary actions and completion of annual performance reviews working in collaboration with the CICP director. A component of this position is to facilitate the design and development of wellness programs and educational offerings that are culturally- sensitive and linguistically appropriate to enhance the health and wellness of Members of OHSU Health Services. The Manager works in partnership with the multi-disciplinary care team in identifying areas of educational focus in the design and development of programs to be provided. The Manager in coordination with the Community Outreach Specialists helps to facilitate community-based education and assists the multi- disciplinary care team to coordinate and provide holistic services for Members/Families that will improve health outcomes, improve engagement with community resources, and empower Members to fully participate in the health care system.

Function/Duties of Position

  • Provide oversight of the Care Integration/Community Outreach Specialist/ Panel Coordinator team and when needed face-to-face and telephonic outreach in assisting the Member to access services and resources in navigating the health care system. May conduct home and community visits on an as-needed basis to assist Members/Families with complex medical/social needs. Conduct screening and outreach to identify social determinant barriers and health risk, coordinating with the Care Integration and Coordination multi-disciplinary team in addressing needs consistent with the Member’s plan of care. May perform initial and general assessments of Members coming into the OHSU Health Services to help in determining future care integration and coordination needs and potential support resources. Coordinate services being provided, and educational programs being presented. Primary responsibility for employee productivity in meeting Program engagement metrics. Oversight of recruitment and employee performance to include, interviewing, on-boarding, coaching, counseling, disciplinary actions and completion of annual performance reviews.

  • Facilitate the design and development of wellness programs and educational offerings that are culturally-sensitive and linguistically appropriate to enhance the health and wellness of Members of OHSU Health Services. Partner with the multi-disciplinary care team in identifying areas of educational focus in the design and development of programs to be provided. Facilitate the development and delivery of culturally congruent, trauma informed health education classes, presentations, programs and written materials for a variety of audiences to include health care providers, community members, Members and Families. Assist in development and delivery of systems and services that encourages Member and Family engagement and health behavior change. Innovations in this area may include community-based interventions, home visits, and family system intervention. Work closely with OHSU Health Services leadership to actively assess the need for modifications and/or development of new initiatives within the program; and collaborate with OHSU Health Services leadership in the development of budgetary projections, providing direct day-to-day operational oversight of budget expenditures. Facilitate coordination of the Community Outreach Specialists in the delivery of community-based education and assist the multi-disciplinary care team to coordinate and provide holistic services for Members/Families to improve health outcomes, improve  engagement with community resources, and empower Members to fully participate in the health care system.

  • Facilitate Health System integration through cross-site communication across the Integrated Delivery System and within the OHSU Health Service Area. Coordinate with the leadership team to serve as a point of contact for Wellness Promotion and Outreach Program areas working closely with providers, facilities, clinics, and agencies to help facilitate the work of Wellness Promotion and Outreach Program that will assist Members to navigate a complex healthcare system. Foster a positive and professional relationship and act as a liaison with internal and external customers to ensure effective working relationships.

  • Monitor and evaluate the services and community-based resources necessary to respond to the Member’s individual health care needs. Ensure the OHSU Health Services Community Resource Directory is kept current. Ensure Specialist Team members are trained and utilizing the Connect Oregon platform for referral to community-based resources.

  • Routinely audit program staff to ensure proper documentation of all Member interactions and Member engagements in the EMR to ensure a longitudinal record is maintained of all Member interactions. Collect clinical and engagement data and enters information into the electronic medical record (EMR) for tracking and metric purposes. Maintain medical record confidentiality at all times through the proper use of computer passwords, maintenance of secure files; adherence to HIPAA guidelines and policies. 

  • Foster positive and professional relationships and acts as a liaison with internal and external customers to ensure effective working relationships. Developing and maintaining an effective working relationship with community partners to increase successful referrals, availability of resources, and interagency innovation.

Required Qualifications

Education:

  • Bachelor’s degree.

Experience:

  • Three (3) years relevant work experience in the field of wellness promotion and/or health education in a mission-driven organization. with progressive leadership experience with a program and/or team. One of the years being work experience with underserved populations with complex medical and social needs.

Preferred Qualifications

Education:

  • Master’s degree in a health related field.

Experience:

  • Experience in community outreach services providing health information, advocacy, social support, and assistance engaging with the health care system.

Job Related Knowledge, Skills and Abilities (Competencies):

  • Knowledge and experience working within Epic as the electronic medical record. 
  • Knowledge of adult learning and behavior change principles.
  • Knowledge of community-based resources.
  • Bilingual in a language prevalent in the local community.

Additional Details

Monday through Friday work schedule, day time hour’s variable to accommodate meeting schedules and availability to members. Position is field-based with the employee working remotely from a home-office location that meets the criteria established by OHSU for teleworking. A portion of the position may require time in the clinic practice setting and in the community setting to fulfill the essential functions of the position; interaction extensively by phone or in person with Members, providers, internal and external customers; minimal travel within the tri-county area.
