Department Overview

The purpose of this position is to collaborate with faculty, staff, and patients to improve patients' health. The pharmacist will provide collaborative drug management services, complete medication reviews, medication reconciliations, and other clinical pharmacy services in addition to traditional dispensing services and other services as required. The pharmacist will assist medical personnel and patients with drug information. the pharmacist will help provide direction to pharmacy technicians and pharmacy students.

Function/Duties of Position

  • Interprets and evaluates provider's orders and prescriptions for accuracy, completeness and appropriateness.  Recognizes therapeutic problems, including drug interactions, contraindications, improper dosing, and precautions of therapy and toxicities. Consults with practitioner when problems or adverse effects are identified. Appropriately reports all adverse drug reactions, drug defects and medication errors.
  • Counsels and trains patients regarding appropriate use of their medications (i.e., purpose, actions, dosage, side effects, storage, precautions etc.)
  • Prepares, labels, and controls all medications for patient use. This includes assuring proper drug storage in all areas where medications are dispensed or stored. Maintains appropriate records as required by law and hospital policy.
  • Participates in and advances patient centered pharmacy programs to include appointment based programs and adherence packaging.
  • Responsible for providing collaborative drug therapy management services (CDTM), complete medication reviews, medication reconciliation services, and other clinical pharmacy services that will help improve patient care and outcomes. Assists with devising, developing and operationalizing programs to optimize medication use and achieve continuous quality improvement.

Required Qualifications

  • B.S. Pharmacy or Pharm.D.
  • Board Certification in Ambulatory Care Pharmacy, Pharmacotherapy, or Psychaitric Pharmacy, or willingness to achieve Board Certification within one year of hire or upon beocming eligible.
  • Drug therapy in patients described in Position Purpose (dosage, indications, side effects, etc.)

  • Contemporary medication use systems (interpret prescribers’ orders, prepare and dispense medications, etc.)

  • Excellent interpersonal communication skills, Systematic problem-solving skills, ability to work independently with minimal supervision

  • Successfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance, and OHSU policies

  • Oregon State Board of Pharmacy License

Preferred Qualifications

  • Board Certification in Ambulatory Care Pharmacy, Pharmacotherapy, or Psychiatric Pharmacy
  • Clinic based practice, previous experience managing patients in collaborative practice agreements.
  • Oregon State Board of Pharmacy Preceptor License
