This position is based in the Oregon Office on Disability and Health (OODH), a CDC-funded program to promote health equity and well-being of people with disabilities. The purpose of this position will be to coordinate and manage the Linkage Project, a program that links adults with intellectual and developmental disabilities (IDD) to needed health care and health promoting services. Primary duties of the position include: 1) lead design and implementation of the Linkage Project, including public relations activities, 2) collaborate on evaluation the Linkage Project program in meeting goals and present findings of Linkage Project to professional audiences, 3) assess interests of disability communities, multiple funders, partner organizations, and other interested parties into design of the project, and 4) supervise program staff who address barriers to health care and other health services to link individuals to needed services, and contribute to linkage activities as needed. Experience with project coordination and management, care coordination and/or navigation that involves disability service and health care systems, and working with adults with intellectual and developmental disabilities is required. Bilingual skills in Spanish and English are a strongly preferred qualification.
