OHSU’s Improving Addiction Care Team (IMPACT) is an innovative hospital-based addiction care team that meets and engages patients during a reachable moment of hospitalization, initiates and coordinates addiction care, and supports linkages to community care for substance use disorders (SUD). IMPACT is a partnership with OHSU and community partners and includes an inpatient addiction medicine consult service at OHSU and rapid-access pathways to community addiction care after hospitalization.

 

The IMPACT Nurse Professional Practice Leader (PPL) will provide patient-centered, patient-directed harm reduction education (e.g. counseling re safer injection, overdose prevention) and kits (e.g. alcohol wipes, clean syringes/pipes) and will serve as a SUD and harm reduction expert and specialty resource for hospital staff and nurses. The IMPACT PPL will work in partnership with the consult team and nursing units to identify patients, offer support and provide targeted harm reduction strategies. The IMPACT PPL may see patients that the consult service does not see, both because of patient preferences (not all patients want to see IMPACT), service availability (e.g. weekends when IMPACT may be unavailable), and consult service capacity. This position will be responsible for managing and coordinating harm reduction health initiatives at the institutional level in partnership with IMPACT leadership and will be responsible for harm reduction education efforts for nursing and hospital staff.

Care Improvement

• Promotes an integrated and interprofessional approach to patient care for hospitalized patients with SUD
• Works closely with all members of the IMPACT Addiction Medicine service using best practice clinical protocols and guidelines to achieve optimal patient outcomes
• Identifies hospitalized patients with SUD at OHSU for participation in safer use, overdose prevention, harm reduction, and sexual health education.
• Reliably communicates pertinent patient information to both members of the IMPACT consult service and hospital personnel.
• Engage hospitalize patients with SUD and evaluate their interest in learning safer use strategies relating to their SUD 
• Develop and deliver patient education on safer use, overdose prevention, and sexual health promotion practices to interested hospitalized patients with SUD.
• Provides patient care in accordance with registered nurse privileges granted by OHSU. 
• Maintains the confidentiality and privacy of each patient, promote a positive relationship with all health team members and support the policies and procedures of the University Hospitals and Clinics. 

Performance Improvement

• Uses data to identify areas for improvement based on assessment and current states.
• Monitors and uses data to determine patient care quality improvement objectives.
• Evaluates the practice environment in relation to existing evidence, research, standards, and care delivery best practices.
• Designs and implements quality improvement strategies to ensure nurse sensitive outcomes meet or exceed established goals.
• Evaluates the impact/benefits of organizational, nursing, and department initiatives, and communicates learning to disseminate best practices.
• Identifies areas of risk and makes suggestions for reducing risk and improving outcomes.
• Investigates concerns and errors to understand occurrences of errors or adverse events.
• Assists in the ongoing development and expansion of the IMPACT Addiction Medicine Consult service. 

Strategic Planning

• Partners with others to develop a program strategic plan that aligns with the division strategic plan and vision
• Collaboratively formulates program objectives, goals and specific strategies related to initiatives. 
• Develops short and long-term goals that identify the target conditions. 
• Coordinates the implementation of interventions to meet program-level goals.

• Mentors and coaches nurses to self-reflect on their practice against professional standards and self-direct their ongoing professional development.  
• Monitors, evaluates, and communicates the application of professional practice standards in initial and on-going competencies, professional practice evaluation processes, and policies and procedures.   
• Collaborates with leadership to facilitate behaviors that impact safety, quality, patient satisfaction, and employee satisfaction. 
• Consults with Clinical Specialist Leaders and members of management to identify individual professional practice issues and engages with clinical nurses to develop improvement approaches. 

• BSN or higher degree in nursing AND Master’s degree in nursing or related field (e.g. MBA with Healthcare Administration concentration, Public Health)
• Five years of experience as a registered nurse
• Experience in complex organization environments.
• Ability to effectively manage multiple conflicting priorities.
• Proficient with word processing, computational spreadsheet (e.g., Excel), and presentation software.
• Unencumbered Oregon RN License
• BLS for healthcare provider
• Nursing administration or specialty practice certification within 3 years of assuming position
• Ability to perform the job duties with or without accommodation.

• Master’s degree in nursing leadership or nursing education
• Experience in nursing management.
• 1+ years of management experience in clinical care.
• Participation in Hospital SUD ECHO series or equivalent SUD training;
• Experience working effectively with IMPACT staff and patients
• Experience in work using principles of trauma-informed care and harm reduction
• Professional certification in nursing leadership-Nurse Executive/Administrator or Professional Development
• Professional certification in addictions nursing
