Department Overview

The transplant APP working as a member of an inter-disciplinary team is integral to Clinical Transplant Services. The APP assists in facilitating discharge for transplant patients, providing education, and works closely with staff members of the transplant team (RN's, physicians, social worker, financial counselor, psychiatrists, etc.) and has the primary responsibility for providing direct and indirect professional care for transplant patients both in the inpatient and outpatient setting.

 

The purpose of this position is to coordinate and support the needs of inpatient and clinical support operations for the Kidney Transplant Program. The NP/PA works in close partnership with team of physicians, advanced-practice providers, RNs, surgical team and hospital personnel to provide excellent clinical care and customer service in the inpatient and outpatient settings. The NP/PA works to assure a smooth transition of patient care and education between the inpatient and outpatient arenas in order to decrease length of stay, improve patient outcomes and increase patient satisfaction. The NP/PA provides modeling for staff and residents regarding excellent internal and external customer service and teamwork. The NP/PA is credentialed by the Medical Board of OHSU to practice in the hospital setting and to perform procedures as authorized by the medical board and within the scope specified by the respective licensing board.

 

Includes providing high quality customer service to both external and internal customers (patients, transplant coordinators, referring providers, internal providers, and other OHSU faculty and staff) that meets or exceeds the service standards for the health care industry. This duty includes prompt and professional communication efforts, face-to-face customer contact skills, crisis management, use of available information technology, standard complaint processing, flexibility in coverage of internal service needs, and continuous application of process improvement methods and skills.

Function/Duties of Position

Clinical Responsibilities

  • Work in partnership with faculty physicians and residents to provide excellent clinical care and customer service.
  • Work to assure a smooth transition of patient care and education between inpatient and outpatient care.
  • Assist staff, residents, faculty, in continuous quality improvement and decreasing length of stay. Includes, but not limited to participation in rounds with faculty and residents, formulating care plans, charting clinical orders and progress notes, supervising and implementing timely discharges.
  • Will field telephone calls regarding all kidney and pancreas patient issues, as needed, address clinical problems, and fill prescriptions. Will follow-up on ordering and checking labs and imaging studies on our transplant recipients. Will promote and maintain the health of service for transplant outpatients through the use of educational materials and counseling of patients and their families.
  • Completes timely and accurate documentation in Epic with an awareness of reimbursement issues.
  • Communicate to all referring providers and primary care practitioners as needed.

Patient Education

  • Participate in patient education in both the inpatient and outpatient setting
  • Providing telephone advice and follow-up phone calls as needed regarding hospitalization and/or clinic visits.

Protocol Development and Revisions

  • Will lead or assist in ongoing protocol development and implementation in conjunction with faculty physicians and other team members.
  • This may include clinical plans of care and/or protocols for service patients with disease processes/ diagnoses/ procedures; participate with house staff and fellow hospital/unit/coordinator staff in evaluating results of protocols and continually working toward improving effectiveness of it.

Professional Development through attendance and/or participation at national/regional conferences.


Other projects as assigned by manager and department director.

Required Qualifications

  • Master’s Degree 

  • Ability to multi-task and prioritize.

  • Ability to work as a team member in an interdisciplinary setting.

  • Demonstrate effective interpersonal and communication skills in verbal, written, and electronic media.

  • Computer/word processing skills

  • Board Certification, Current Oregon NP or PA License, Prescriptive authority/DEA licensure, NP candidate with acute care certification

  • BLS from American Heart Association

  • Compliance with Code of Conduct, Respect in the workplace and Applicable policies, procedures and agreements related to position, department or OHSU as a whole

  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

  • >2 years experience in inpatient Med-Surg prefered, but new graduates encouraged to apply

  • Epic electronic medical record system

  • WA NP or PA License

     

Additional Details

  • Varies 0630-1900 Monday-Friday as negotiated with management. May be required to work off shift hours, attending early morning meetings, rounds, or conferences, as decided by clinical needs of the specialty.
  • May require additional skills to be acquired during the probationary period to adequately care for the specified patient population. Requires prioritization of multiple projects, activities, and assignments. Frequent interruptions in an environment of frequent change and fluctuations. May be exposed to excessive auditory and visual stimulation. May be required to attend out of town conferences according. May be exposed to body fluids, tissue, possible radiation exposure, and infectious processes. Subject to computer work for several hours, as needed. Able to utilize all patient care equipment and office equipment within work area.
