Department Overview

Family Medicine at Richmond Clinic is a primary care Family Medicine clinic and Federally Qualified Health Center (FQHC).   The patient population includes pregnant people, babies, children, adolescent, adults, and geriatric.  The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions.  These include, but not excluding: oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management. 

 

Work Location

  • In-clinic, with Hybrid schedule available after completion of an orientation period
  • Position eligible for WFH opportunities 6 month after date of hire

Function/Duties of Position

 

 Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where pts. are reviewed for Care Management, Care Coordination, follow up for discharge planning, and triage/advise for medical conditions.  Care Coordination and Disease Management are a high priority aspect of the job.  Nurses work in pods within a care team of Providers, Pharmacists, and Behavioral Health clinicians.

  • External Candidates: Vacation, paid or unpaid leave requests up to 8-weeks from the start date are not permitted. All other vacation, paid or unpaid leave requests are subject to compliance with the Collective Bargaining Agreement between ONA and OHSU.
  • Internal Nurse Candidates: All vacation, paid or unpaid leave requests are subject to compliance with the Collective Bargaining Agreement between ONA and OHSU. Pre-scheduled vacations or leave plans must be disclosed prior to consideration for hire to determine the start
  • Experience:

    • New Grad: Must have 90 hours of direct patient care through a practicum or capstone in a community health setting.
    • Experienced RN: Must have two years of direct patient care experience within the last three years.

Required Qualifications

  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA)
  • BLS must not expire during orientation
  • Must be able to perform the essential functions of the position with or without accommodation
  • Recent experience working in Primary Care or an FQHC setting
