The OHSU Cardiac Rehab Nurse Coordinator is responsible for coordinating the specialized care for patients with cardiovascular disease in Cardiac Rehabilitation. In this role, the nurse works with Exercise Physiologists, the Medical Director of Cardiac Rehabilitation, and the Advance Practice Provider in Cardiology in the provision of patient care-assessment, reassessment, planning, implementation and evaluation of the plan of care. The nurse provides patient education and counseling for identified risk factors and promotes life-style modifications pertaining to cardiopulmonary disease and related health issues. The nurse especially supports care, education and interventions outside the scope of the Exercise Physiologists, such as medication adherence, lipids, smoking cessation, comorbid conditions, etc. In this position, the Coordinator will collaborate in the development, implementation and maintenance of all aspects of care for patients in Cardiac Rehabilitation in collaboration with the multi-disciplinary care team.
The Coordinator will utilize discretion and independent judgment to facilitate seamless care from referral to follow-up for patients who are candidates for Cardiac Rehabilitation. The Coordinator will apply the nursing process to the provision of care for this subset of patients; and together with the interdisciplinary team, establishes a plan of care that starts at initial interventional evaluation and follows the episode of care. The Coordinator will work with the patient and family to provide teaching, education and support throughout the Cardiac Rehabilitation process. The RN Coordinator oversees the Cardiac Rehabilitation program. This includes establishing and developing workflows organization wide for access and education. The coordinator leads and develops the Cardiac Rehabilitation's internal staff education/competency plan, quality improvement program, and together with the multidisciplinary team ensures compliance with accreditation requirements (DNV and the American Association for Cardiac and Pulmonary Rehabilitation). The Coordinator supports professional knowledge and healthcare management through effective interactions and communications with clinicians, healthcare administrators, community agencies and other professionals.
