Reporting to the Operations Manager Specialty Pharmacy Services this position will support the Pharmacy
Refill Authorization Program (RAP) as well as OHSU Pharmacy Benefit Management Services and Specialty
Pharmacy Program ambulatory clinic services in a relief capacity depending on individual service line needs.
RAP: This position is responsible for the provision of specialized clinical and/or MTM services to patients in a
community/ambulatory setting. This position is responsible for providing refill authorization services to
ambulatory clinics, supervision of support staff and ensuring compliance with all legal, regulatory and
institutional directives. This pharmacist will interact directly with prescribers and clinic staff to develop an
interdisciplinary approach to patient care. Additionally, the pharmacist will be responsible for the development
and implementation of pharmacy program opportunities and will be expected to contribute to the financial
viability of the program, department and institution.
The PBM Pharmacist provides clinical support to OHSU Pharmacy Benefit Management Services. Support
includes but is not limited to formulary management, coverage determinations, Pharmacy and Therapeutics
Committee support, responding to clinical inquiries, and implementing clinical programs, as outlined in the
PBM Pharmacist position description.
Support of the Specialty Pharmacy Program and ambulatory clinics provide the services of a licensed
pharmacist which includes the provision of specialized clinical and/or comprehensive medication management
services to patients in the ambulatory as outlined in the Specialty Ambulatory Clinic Pharmacist position
description.
Interprets and evaluates refill authorization requests for accuracy, completeness and appropriateness. Recognizes therapeutic problems including drug interactions, contraindications, improper dosing, and precautions of therapy. Recognizes actual or potential problems and provides therapeutic recommendations for their resolution.
Provides medication therapy management, medication reconciliation, allergy review and collaborates with providers to ensure safe and appropriate therapy. Appropriately reports all adverse drug reactions, drug defects, and medication errors.
Participates in committees supporting refill authorization protocols and Epic development of tools to support workflows.
Assists in the development, maintenance and implementation of pharmacy programs to support overall services for patients.
Provides necessary support for service level agreements with ambulatory clinics.
Conducts therapeutic interchange under the collaborative drug therapy management program. May review authorization requests to identify and make recommendations on formulary alternatives that support cost effective
prescribing. Collaborates with clinic and support staff to resolve authorization issues.
Assists with devising, developing and operationalizing programs to ensure proper use of medications and continuous quality improvement.

The pharmacist will experience competing demands on their time from the volume of requests and will need
to effectively multi-task and prioritize demands. The pharmacist must deal with time constraints and need to
exercise clinical judgement on a daily basis. The expectation will be to provide clinic staff and patients with
excellent customer service. Additionally, within the healthcare setting there can be a significant lack of
control over the work pace, with frequent interruptions (work is often dictated by external factors) that may
lead to mental fatigue or stress.
