Department Overview

This position coordinates the pharmacy review and categorization of medication-related safety events that

have been submitted to Patient Safety Intelligence (PSI), supports pharmacy supervisors/managers and

hospital committees by writing and distributing requested reports from the PSI system, recommends changes

to improve the safety of systems for patients, and assists in the training of students and residents.

Function/Duties of Position

  • Reviews and categorizes PSI reports
  • Coordinates the completion of overdue PSI reviews with managers and supervisors

  • Writes and distributes requested reports from the PSI system
  • Assists in the medication safety education and training of students/residents
  • Attends the Medication Safety Committee, Pediatric Medication Safety Committee and City-wide Medication Safety Committee

  • Assists in collecting and analyzing medication safety related data for quality review and improvements

Required Qualifications

  • A current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
  • Hospital pharmacy practice
  • Medication safety principles Contemporary medication use systems including automation Interpersonal communication (formal and informal), including presentations Project management skills including organization of data and assignments, prioritization of competing demands and adherence to deadlines Systematic problem solving and data analysis Computer skills including use of the EMR, and common word processing, spreadsheet, and databases

Preferred Qualifications

  • Ambulatory and retail pharmacy practice, medication safety practice
  • Specialist Certifications (CPPS, Medication Safety Certificate, etc)

Additional Details

The pharmacist works primarily independently. There is significant computer time that may result in ocular fatigue. The work can become repetitive requiring self motivation to maintain attention to detail.
