Department Overview

The purpose of this position is to provide the services of a licensed pharmacist which includes the responsibility for the safe and proper distribution and use of drugs patients who range in age from neonates, infants less than one year of age, children and adolescents ages 1-18, adults ages 19-65 and the elderly over age 65.

Function/Duties of Position

  • Interprets and evaluates physician's orders and prescriptions for accuracy, completeness and appropriateness. Recognizes therapeutic problems, including drug interactions, contraindications, improper dosing and precautions of therapy and toxicities. Consults with practitioner when questions or problems are identified.

  • Monitors therapeutic response and adverse effects of drug therapy. Consults with practitioner when question or problems are identified. Appropriately reports all adverse drug reactions, drug defects, and medication errors.

     

  • Counsels and trains patients regarding appropriate use of their medications, (i.e., purpose, actions, dosage, side effects, storage, precautions, etc.).

  • Prepares, labels and controls all medications for patient use. This includes assuring proper drug storage in all areas where medications are dispensed, or stored. Maintains appropriate records as required by law and hospital policy.

  • Counsels and trains patients in the appropriate use of their medications (i.e., purpose, actions, dosage, side effects, storage, precautions etc.). Participates in drug product selection, counsels prescribers on the selection of appropriate and cost effective alternatives.

  • Researches and evaluates professional literature in response to questions and requests and provides drug informatin to clinic staff and/or OHSU patients.
  • Complete initial and continuing training and competencies for required FWA and DMEPOS programs.

Required Qualifications

  • A current Pharmacist’s license issued by the Oregon State Board of Pharmacy.

  • B.S. Pharmacy or Pharm.D.
  • Community pharmacy practice.

Job Related Knowledge, Skills and Abilities (Competencies):

  • Drug therapy in patients described in Position Purpose.
  • (Dosage, indications, side effects, etc.) Contemporary medication use systems (interpret prescribers’ orders, prepare and dispense medications. etc.)
  • Systematic problem solving skills,ability to work independantly with minimal supervision Excellent interpersonal communication skills

Preferred Qualifications

  • Advanced pharmacy practice residency
  • Inpatient hospital pharmacy practice

Additional Details

The pharmacist will experience competing demands on their time from clinic staff and/or patients and will

need to effectively multi-task and prioritize demands. The pharmacist must deal with intense situations on a

regular basis. The expectation is to provide patients with exceptional customer service at all times.

Additionally, within the healthcare setting, there can be a significant lack of control over the workplace,

with frequent interruptions (work is often dictated by external factors) that may lead to mental fatigue or

stress.
