Department Overview

Family Medicine at South Waterfront is a community-based clinic offering primary care and specialty services for all types of patients. We treat everyone--babies, children, adolescents, adults (including moms-to-be), and seniors. Our services include prenatal care (including deliveries), women's reproductive health, behavioral health care and counseling, transgender care, pediatric care, travel immunizations, podiatry and foot care clinics, sports medicine, Osteopathic Manipulative Treatment (OMT), Office Based Medication Assisted treatment (OBAT), acupuncture, minor procedures, and health maintenance with preventive care. Between 175 and 275 patients are seen daily by our 40 plus providers assisted by 60 plus staff members.

Function/Duties of Position

The key duties of the Clinical RN include phone triage, managing nurse visit schedules, anticipating patient needs, and providing clinical support within ambulatory workflows. The RN advocates for patients and families, ensures efficient, safe, and cost-effective care, acts as a liaison between referral and clinic providers, and participates in interdisciplinary teams and clinic operations aligned position descriptions expectations.  This may include active care coordination and management of a high-risk ambulatory patient panel.

 

Required Qualifications

Department Specific Requirements (experience, competencies, certifications, student placements, etc.):

  • New Grad:
    • Must have 90 hours of direct patient care through a practicum or capstone in a community health setting.
  • Experienced:
    • Must have 2 years of direct patient care experience within the last 5 years. 
  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA)
  • BLS must not expire during orientation
  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

  • Current clinical nurse experience in an ambulatory care setting including telephone triage. 
  • Experience working in Primary Care setting within the last three years.
  • Nursing professional certification in Ambulatory Nursing.

  • Either Transition to Practice (TTP) Program Residency Learning Pathway or Orientation experience for about 10-26 weeks depending on the care area. An employment service agreement may apply.
