OHSU Division of Geriatrics and Internal Medicine is seeking a Behavioral Health Consultant (BHC) to join an innovative, multidisciplinary team of clinicians and researchers in implementing the PATHS-CC (Peer- Assisted Telemedicine for Health in Substance Users-Comprehensive Care) program. This program is designed to provide on-demand telemedicine access for mental health and substance use care to people who use drugs across Oregon, particularly those impacted by homelessness, serious mental illness (SMI), co- occurring disorders (COD), and hepatitis C (HCV).
The PATHS-CC initiative delivers same-day HCV/HIV testing and diagnosis, followed by integrated behavioral health services, with the goal of improving health outcomes in highly burdened communities and developing scalable models for HCV elimination.
The Behavioral Health Consultant will be a clinically licensed provider responsible for conducting same- day telemedicine behavioral health assessments and interventions, including for individuals with SMI, CODs, and
substance use disorders (SUDs). The BHC will work closely with a care team, including RN Care Managers, Mental Health Providers, Peer Support Specialists, and SUD treatment providers, to ensure rapid and coordinated access to care.
The ideal candidate will bring experience in integrated behavioral health, a strong commitment to serving underserved and vulnerable populations, and the flexibility to work in a dynamic, telemedicine-driven care
model.
Providing same-day telemedicine visits for up to 100 participants per year, addressing behavioral health concerns and initiating treatment or referrals as appropriate.
Participating in telemedicine-based triage and suicide prevention in collaboration with RN Care Managers and other behavioral health professionals.
Utilizing evidence-based approaches, such as motivational interviewing and cognitive behavioral therapy, to support participant engagement and retention in care.
Ensuring ongoing follow-up and facilitated linkage to local behavioral health and SUD services for continued care.
A Master’s Social Work (MSW) required.
Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required for employees who practice clinical social work.

This is a .50 FTE position with a daytime schedule and may include weekend or evening hours. This position will be a remote.
