The Social Work Specialist assists in the development of plans to provide continuity of care and enable patients and families to achieve maximum physical, social and emotional health.
The Center for Women's Health is a Center of Excellence in Women's Health care. We have a wide variety of sub-specialties that provide care within the Center for Women's Health. Our patients will often see multiple providers within this same physical space for healthcare needs across their lifespan. This position will support the needs of those patients, working with Providers/Medical Assistants/Registered Nurses to help facilitate the correct care for those patients in need.
Two years of social service work experience providing supportive counseling to clients on a one-to-one basis which includes full responsibility for handling each case plan.
A Bachelor’s degree, or three more years of experience which shows: a) knowledge of casework methods and techniques; b) knowledge of patient’s family’s rights; c) knowledge of medical terminology; and d)knowledge of implications of the effect of illness, injury and/or disability on patients and families.
