Department Overview

Doernbecher Pediatrics & Adolescent Health clinic specialize in providing primary care for newborns, infants, children and adolescents. Services include well child exams, immunizations, sports physicals, developmental assessments, care of acute and chronic illnesses as well as coordination of care for chronic medical conditions and mental health issues.

Function/Duties of Position

 

The nurse in this position will provide comprehensive care to pediatric primary care patients in an ambulatory setting, including the provision of education, care management, and oversight and response to phone triage requests from patients and families as well as providing in clinic support. The Registered Nurse provides complex care coordination, extensive phone triage, and clinic management. In addition, the nurse will coordinate multi-discipline care plans for patients as needed. Determines the patient’s ability to engage in actions that promote health and participation with their medical team. Together with the patient and primary care provider, the nurse will create a comprehensive and evolving plan to achieve shared health goals.  Outreaches to patients to coach, educate, monitor progress, and re-evaluate plan regularly. Maintain contact with patients to encourage healthy behaviors, identify barriers to care, and to assist with access to needed resources or interventions.

 

  • Ability to perform Ambulatory RN assignment(s) that may vary by week, day, or partial day depending on status of need as determined by the practice manager and/or medical director.
  • Communication skills including fluency in oral and written English.
  • Navigate information technology associated with the position.
  • Use Electronic Health Record information and associated tools.
  • Record, track and assign duties and processes to ensure appropriate billing and revenue generation, when applicable.
  • Facilitate care coordination with other providers and services to complement Pediatric Specialty care and meet holistic needs of the patient and family.

Required Qualifications

  • Minimum 2 years of pediatric nursing experience
  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS from AHA required. New hires will be enrolled and required to complete during orientation.
  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

  • National Certification in a pediatrics, preferred.
  • Experience working in Ambulatory Primary Care Pediatric practice, preferred.
  • Experience with pediatric telephone triage, preferred.
  • Experience in Care Management
  • Experience in working with socially and medically complex patients and their families
  • Experience with nursing primary care phone triage and communication with outside community resources
  • Fluency in English and Spanish
  • Experience in childhood immunizations
  • Experience in asthma education
  • Experience in patient education

 

Additional Details

Length of Orientation – Experienced Nurse

  • External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
  • Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
