Department Overview

The purpose of this position is to ensure the integrity and accuracy of all patient care health information stored in OHSU’s electronic medical record. This role performs a variety of functions related to the retention of Protected Health Information (PHI) documentation, and its security. 

 

Health Information Management staff are expected to support and actively engage in OHSU's Performance Excellence System (OPEx) as a foundational principle and method for improving department quality and efficiency and increasing the value of patient care through the continual improvement of Electronic Health Record documentation.

Function/Duties of Position

Indexing

Performs accurate and efficient indexing and classification of electronic health record images through the point of care Solarity and OnBase applications.

  • Indexing includes a thorough quality review of each image for specific criteria: correct capture class, documentation accuracy, documentation knowledge, clarity of image, accuracy of EHR location, proper identification on image itself; (patient name, date of birth, medical record number, &/or contact serial number) and indexing to the correct patient.
  • Maintain Quality: Complete any corrections logged on personal error report and work with the Document Imaging Manager regarding quality trend or general errors for improvement.
  • Maintain current SLA’s/TAT for all workbaskets.
  • Follow Policies, Standard Operating Procedures and Job Aides for workflow processes.
  • Log and report errors to the Document Imaging Manager.
  • Bring issues and findings to the Document Imaging Manager.
  • Track and maintain communication shared regarding all updates and changes that pertain to the Document Imaging workflow that is shared via the Document Imaging Manager and others.

Other

  • Solarity assigned functions.
  • OnBase assigned functions.
  • Solarity and/or OnBase Importing.
  • Importing records that are sent by email.
  • POLST – processing POLST to Epic/Vynca.
  • Epic Pools: Save to Chart, Patient Entered Attachments and PDF’s, Patient Entered Video, and others as assigned.
  • Paper Workflow: Triage/Scanning, scan requests, etc.
  • Follow all Policies, Standard Work and Job Aid around Triage, including:
    • Opt-Out – indexing and processing data into Epic.
    • CD’s -importing electronic images accordingly.
    • Other assigned on Triage days.
  • Assigned project work.
  • Attend Meetings – Team Daily Readiness Huddles, 1:1’s, etc.
  • Attend assigned employee development/Team classes, etc.
  • Track and maintain communication emails for documents that need to go to the correct teams (ROI, etc.).
  • Track and maintain communication shared regarding documents, updates and changes that pertain to the Document Imaging workflow that is shared via the Document Imaging Manager and others.
  • Other duties as assigned.

Required Qualifications

  • High School diploma or GED; and Two years of general medical records processing and document imaging experience; OR
  • An Associate’s degree or certificate in Health Information Management, office occupations or office technology and one year of general medical records processing and document imaging experience; OR
  • An equivalent combination of training and experience. 
  • Previous work experience working with Electronic Health Record applications.

Knowledge, Skills, and Abilities

  • Knowledge of medical terminology. 
  • Must be detail focused, demonstrating organizational abilities to follow directions and perform daily tasks efficiently.
  • Multi-tasking skills, adept to changes that are sudden and last minute.
  • Proven ability to visually distinguish between good/poor quality images easily.
  • Proven ability to perform both quality and quantity in daily assigned tasks.
  • Adept at process troubleshooting, requiring ability to backtrack work processes, step-by-step.
  • Demonstrable computer knowledge, & manipulate multiple software applications in, a single session without difficulty.
  • Experience working with office-related equipment, including but not limited to - High/Low Speed scanners, Fax machines, Xerox copiers, and paper cutters, document shredders, portable CD attachments, printers, laptops, and monitors.

Preferred Qualifications

  • Previous experience working in a large tertiary health care facility.
  • Experience with Solarity, OnBase, and Epic.
