The Care Coordinator 2 is an integral member of the neurology care team, partnering with clinicians, nursing staff, patients, families, and community organizations to support seamless, patient-centered care. This role serves patients with a broad range of neurologic conditions by coordinating referrals, diagnostic testing, specialty services, medication-related workflows, and transitions of care across multiple settings, including outpatient clinics, hospitals, rehabilitation facilities, and community-based resources. The Care Coordinator II also serves as a key communication link among patients, caregivers, providers, and interdisciplinary team members to promote continuity of care and timely follow-through on treatment recommendations.
