The Foregut surgery RN Care Coordinator is a member of a multi-disciplinary team with primary responsibility for direct and indirect professional nursing care of medicine patients that have complex esophageal diseases, many of whom have oncological diagnosis.   The Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the foregut surgeons in the ambulatory / clinic setting.  Foregut surgery is a specialized field where the team nurse gets to know many of the patients closely and plays a key role in patient education and support.  The practice requires management of both benign and malignant expertise. The nurse will become familiar over time with the various treatment options and ongoing complex care.  The nurse will also perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews.  - Responsibilities include the care coordination and case management of individuals in the outpatient clinic - S/he will provide clinical support services to faculty and APP in the foregut surgery section - The nurse Coordinator will be responsible for coordinating comprehensive care for patients potentially requiring advanced procedures and will be a key contributor to ensuring that patients are guided through all stages of their treatment. - This patient population can be labor intensive given their often-oncologic diagnoses and multi-disciplinary care. This translates to increasingly complex care coordination. - The nurse coordinator will be the core of a multi-disciplinary patient care team, which includes physicians, APP, RN, MA, and PAS-R support. - The nurse coordinator will have specialized knowledge and skills in procedures, cancer care, and gastrointestinal disease management - This position serves as a coordinator for patients undergoing sequenced multidisciplinary treatment, and at times patients who travel from long distances from home - Responsibilities include but not limited to care coordination, direct patient assessment, triage, patient education, medication management, discharge and admit planning, and clinical support for the foregut clinic faculty providers. - This position will also participation in quality improvement, medical records review and  will follow existing protocols determining appropriate patient scheduling.

Clinical Service: - Provides anesthesia care within the care team model commensurate with education, licensure, certification, and within limits of experience and expertise. - In collaboration with and under the direct supervision of a medically directing anesthesiologist: - Perform pre-anesthetic assessment on all patients. - Interpretation and treatment of all pertinent lab and assessment data. - Obtain additional lab data, medical testing and additional consultation as appropriate. - Develop and implement an anesthetic plan. - Proper selection and placement of invasive and non-invasive modalities for monitoring and evaluating patients throughout the perioperative period. - Proper selection and administration of anesthetics, adjuvant drugs, fluids and blood products necessary to manage an anesthetic. - Verify that all standard and special anesthesia equipment is operational prior to the anesthetic. - Induction, maintenance and emergence from anesthesia. - Appropriate transfer of care to the postoperative receiving unit. - Communicate clinically significant changes in patient condition on surgical intervention to the medically directing anesthesiologist and jointly develop modifications to the anesthesia plan, as indicated. In conjunction with other care team members, is responsible for: - Using all equipment appropriately and in a safe manner. - Lift, move, and position patients utilizing correct body mechanics - Promotes best practice in all areas of patient care. - Facilitate operating room capacity, case flow and minimize case delays. - Promote infection control and participate in quality and patient safety initiatives. Scholarly Activities: - Participates in clinical research projects, as approved by the APOM Vice Chair for Research, conducted throughout the perioperative period. - Participates in ongoing APOM CQI activities as directed by the Chair or appropriate Vice Chair. Educational Activities: - Responsible for the didactic and clinical training of learners, as approved by the APOM Vice Chair for Education and CRNA site director, throughout the Dept APOM, which may include: - Paramedic students - Flight nurses - Perioperative and ICU nursing staff - Residents and Fellows from departments outside of anesthesiology - Registered Student Nurse Anesthetists - Medical students - Administrative and support staff - Active participation in scheduled continuing education opportunities in the Dept APOM. Administrative Activities: - May participate in additional departmental or hospital administration as approved by the Chair, supervising Vice Chair or Chief CRNA. Other activities: - May engage in public service through activities with OHSU and non-University groups as approved by the Chair.
