Staffing the Columbia Memorial Hospital Emergency Department as assigned by the Chair; participation in education of medical and paramedic students, house staff and nurses; participating in Emergency Department quality assurance activities.

The fellow will gain significant operative experience in all aspects of gender-affirming surgery and is expected to have prior ample experience in microsurgical, craniofacial and aesthetic surgery. Approximately 1 day a week is spent in the outpatient setting seeing patients in clinic, the other 4 days are spent in the operating room. 5 full time fellowship faculty specializing in gender-affirming surgery (3 plastic surgeons and 2 urologists) provides a robust surgical volume. The fellow is required to attend the weekly Plastic & Reconstructive Surgery teaching conference and will present once a year and will also attend monthly multidisciplinary transgender health conference. Complications will be presented at the Surgical Mortality and Morbidity Conference (M&M). The fellow will be provided support to attend the annual USPATH or WPATH conference. If a podium or poster presentation is accepted, the fellow may provided with the opportunity to attend an additional annual conference. The fellow participates in the education and training of medical students, house staff, and the regional medical community. The fellow will take general plastic surgery call at OHSU or community partner hospitals up to approximately 3-4 days a month and 1 weekend every two months.

Imaging - Operates multi-slice CT Scanners to perform exams with accuracy and efficiency, using optimal skills and knowledge of equipment. - Demonstrates knowledge and skill to evaluate anatomy and any related pathologies for presentation to the Radiologist. - Demonstrates knowledge and skill in CT physics & Radiation Safety. - Exercises responsible judgment in recognizing and reporting significant situations relating to patient care. - Prepares and instructs the patient for the diagnostic exam using age appropriate communication skills. - Adjusts scanning factors and patient positioning to eliminate or minimize artifacts due to patient habitus, motion, implants, or monitoring equipment.    - Adheres to the ALARA principle while optimizing the exam to obtain the highest image quality with the lowest possible radiation dose to the patient. - Must be able to perform all ranges of exams after completion of technical competencies. Screen patients for and obtain/record patient’s chief complaint, take accurate history of illnesses/surgeries (Ophthalmic and General), past ocular history, family history, drug allergies, and medications.  Assist physician with completing/transferring patient information to the patient problem list as required per JCAHO.  Perform ophthalmic testing as appropriate per patient.  This includes but is not limited to checking/documenting distance and near vision in adults and children with and without correction, clearly determining from patient history if the patient is needing new glasses on any given visit.  Pupillary assessment to rule out afferent defect and to check pupil size (rule out asymmetry), corneal curvature, corneal pachymetry, corneal topography, color vision testing, must be proficient in performing lensometry including reading lenses with multifocal and progressive adds, power of sphere, axis of cylinder and cylinder power, and orientation of prism and measurement of interpupillary distance.  Perform applanation tonometry, and utilize Tonopen (noncontact tonometry) to check intraocular pressure.  Perform confrontation visual fields.  Must be proficient in refractometry and auto refraction, fogging with astigmatic dials, cross cylinder refinement and measurement of accommodation.   Must be able to perform cover/uncover test, alternate cover test and quantify strabismus.  Must be able to document ocular motility, presence of strabismus or nystagmus.  Must be able to perform Worth 4-dot test and Stereopsis test.  Must be able to test near point of convergence, ductions, versions and vergences.  Must be able to perform Potential Acuity Meter testing, Glare Testing.  Instill dilating and other ophthalmic drops per physician’s instructions.   Perform calibration of automated and non-automated perimeters.  Maintain a working  knowledge of computerized visual field programs, and be able to perform Amsler Grid testing.  Maintain a working knowledge of disease processes that may affect the visual pathway and resultant changes in the visual field due to these disease processes in order to adequately perform Goldmann (non-automated) perimetry (central and peripheral).  Perform SITA and Standard Humphrey (automated) (central and peripheral).     Assist doctors during examination as needed.  Apply eye dressings, protective shields and dispense medications as directed by the physician.  Provide patient instruction, education and assistance as required.  Always be willing to assist patients in clinic areas.  Per standard guidelines, clean exam lanes and minor surgical instruments.  Prepare for the sterilization process, either sterilizing instruments utilizing the sterilizer in the Minor Surgery Room per guidelines or preparing instruments to be sent for sterilization.  Responsible for room maintenance including periodic cleaning of exam lanes.  Keep rooms stocked with appropriate instruments, as well as necessary ophthalmic drops, per physician’s direction.   Other duties:  Take ‘tech calls as needed to assist front desk scheduling team, refill pharmacy requests as per physician guidelines; A postdoctoral researcher position is available in the laboratory of Dr. Sarah F. Andres at the Pape Family Pediatric Research Institute, Oregon Health and Science University, Portland, OR.   We are looking for an enthusiastic, highly motivated, and dedicated Ph.D. scientist who has experience with gastrointestinal biology, extracellular vesicles, or mouse models to join our team.   The postdoc will lead projects centered around novel mechanisms that enhance or reduce intestinal damage during the neonatal inflammatory disease necrotizing enterocolitis (NEC). Ongoing projects include defining therapeutic roles of human milk extracellular vesicles in the neonatal intestine and the use of novel genetic mouse models to delineate post-transcriptional regulation in early intestinal development and NEC.   The lab uses genetic mouse models, human patient samples, including novel 3D patient-derived enteroids, cutting-edge molecular biology, -omics approaches, and imaging techniques to answer critical questions.   In addition to scientific training, the postdoc will also receive personalized mentoring to advance their own career development including networking introductions, goal setting, leadership opportunities, further training in public speaking, project management, grant, and manuscript writing, and chances to review manuscripts and attend conferences.   Our research is highly translational with the ultimate aim of taking bench findings back to educate bedside treatments and practices. Successful candidates will be dynamic, self-starters with a doctoral degree. Strong communication, organizational skills, and the ability to think critically are essential.

PERSONAL & PATIENT CARE AND SAFETY: - Provides for personal and patient care and safety using professional judgment in accordance with written standards so there is no injury to self or others. - Protects patients and others in the area by using proper machine settings and radiation safety standards. - Practices universal and blood and body fluid precautions, and sterile practices according to procedures. - Communicates and uses knowledge of related anatomy and physiology to insure appropriate and effective care to neonate, pediatric, adolescent, adult and geriatric patients. - Maintains awareness of patient status and takes appropriate actions to meet their needs. - Explains the procedure to the patient to assure clear answers to their questions and concerns to allay any anxiety and assure patient confidence. - Reports knowledge of any patient safety concern or occurance to manager and submit occurance to the on-line Patient Safety Net (PSN). Program Administration: - Provides essential customer service to course participants, delegates and interested parties. Responds to inquires via various channels (email, phone, in-person, etc.) Represents CARE department to OHSU community and external audience. Uses OHSU Learning Management System (“Compass” extensively to creates courses/classes, receive and confirm registrations, track attendance & outcomes, as well as other functions.Coordinates with partnering OHSU departments for shared programs. Provides primary registration support and processing for BLS training at OHSU. Distributes teaching and learning materials as needed. - Coordinates with OHSU Simulation, EdComm, Teaching Services and others to schedule classrooms, equipment and resources for CARE courses. Prepares printed materials, classroom, audio/visual and educational equipment for CARE courses. Greets participants, completes check-in/check-out procedures. Cleans and breaks-down classroom stations and equipment. Processes completed course paperwork and evaluations per AHA guidelines. Prepares invoices and coordinates billing with OHSU departments and external partners. Program Documentation: - Assists with CARE AHA TC accreditation, follow-up, tasks and expectations. Activities include, but are not limited, to coordination, scheduling and documentation of meetings, assembly of annual AHA TC report, as well as any other requested information. Assists with AHA Instructor certification, renewal & documentation. Activities include, but are not limited to, paper and electronic records management, preparing AHA forms, AHA card security and printing, providing reports to AHA instructors as needed. Program Development: - Works with the CARE Medical Director, Director of Operations & Senior Educator to develop the CARE course catalog and registration. Documentation and requirements as needed. Establishes and maintains procedures, coordinates projects to accomplish goals and objectives. Assists with the coordination of all CARE courses. Departmental Support: - Orders and maintains course material inventory (textbooks, cards, on-line licenses, etc.). Orders and maintains office supply inventory. Coordinates work orders, CARS & IT Service requests. Other duties as assigned.
