Clinical: Fellows will spend their time with different faculty performing gender-affirming surgeries, as well as other reconstructive urologic surgeries. How this exposure is balanced will depend on fellow interest, previous exposure in prior training, surgical skill, as well as clinical demand. Exposure to other reconstructive surgeries is important for a well-rounded training experience and to gain greater insight and knowledge about reconstructive principles. We anticipate that on average one day a week will be spent in an outpatient clinical setting/research/administrative time. Exposure to outpatient management is critical in building comfort and sensitivity with a vulnerable, underserved, and frequently traumatized patient population.   As this field is very collaborative with other specialties, fellows will have extensive exposure to other disciplines. Fellows will be expected to attend all meetings of the OHSU Transgender Health Program, including monthly committee meetings, monthly multi-disciplinary case reviews, etc. Fellows will attend and be expected to present research and clinical teaching presentations at national and international meetings, in particular the WPATH/USPATH/EPATH meetings, and the annual AUA meeting. Fellows may be asked to give teaching presentations within OHSU and locally, as well.   Dr. Jens Berli, Plastic & Reconstructive Surgery, who leads our Phalloplasty program, plans a fellowship training program for Plastic Surgeons in gender-affirming surgery as well. We envision a formal “exchange” period where the Urology and Plastic Surgery fellows would rotate for 2-3 months with the other service and act as the fellow for that service. This is particularly important in learning about phalloplasty procedures, and learning surgical skills unique to a plastic surgery experience. Fellows will make daily patient rounds and act as a point of contact for residents in the residents’ management of patient care. This may include rounding on weekends at times.   As board-eligible and licensed urologists, Fellows will take call as OHSU faculty at a frequency consistent with other OHSU Urology faculty and be responsible for clinical care as a result of those duties. Fellows may also be called upon to periodically staff patient consults seen and worked-up by the residents. This is important in developing autonomy needed for future practice, as well as financial support of the fellowship position.   Outstanding level of professional performance expected. Professional performance is delivered in a manner that is compliant with the regulation and guidelines of all relevant regulating bodies (i.e. DNV, Centers for Medicare and Medicaid Services, OHSU IRB, NIH and all other state, federal, and institutional regulatory guidelines.) Treats all patients, research subjects, trainees and co-workers with dignity and respect.   Academics: Fellow may be involved in teaching support staff in their area of professional expertise. Teaching is done through assigned lectures to be given during the course of the year and through teaching in the course of performing their own job by modeling best practices, mentoring trainees, guiding learning through delegating graduated and focused levels of responsibility. Must be knowledgeable of and compliant with all applicable regulating bodies (e.g. ACGME competencies for clinical faculty, NIH guidelines for research faculty).   Research: The existing research in gender-affirming reconstructive surgery is relatively limited. This positions us for ground-breaking research in this sub-specialty. Many opportunities for research collaboration exist within the OHSU Transgender Health Program with Plastic Surgery, primary care in both Internal Medicine and Family Medicine and other specialties.

This position will play a critical role in OPIP’s effort to improve the provision of behavioral health for children, with a heavy emphasis on behavioral health services for children birth to five. This position will support a Learning Collaborative that provides training, resources, and implementation supports to integrated behavioral health clinicians in primary care sites. These trainings will focus on assessments and brief interventions that can be done for children overall, but particularly for children in a primary care setting. Secondly, this position will support improvement efforts and facilitation of health systems on how to understand their current specialty behavioral health provider network, how to summarize the network of services, and then how to improve and expand their behavioral health network. Lastly, this position will support OPIP’s effort to develop metric and policy recommendations meant to enhance the breadth and depth of behavioral health services provided to children.    This position is under the supervision of the Director of the Oregon Pediatric Improvement Partnership (OPIP). The position will require some travel across the state and in the regions that OPIP leads efforts.

The Mishra lab aims to unravel the mechanisms underlying the neurovascular dysfunction in neurological conditions, with particular focus on ischemic injury and their contributions to Alzheimer’s disease and related dementias. Current projects in the lab aim to characterize the neuroinflammatory processes, especially the responses of astrocytes and microglia, following mild ischemia that may accelerate pathology in Alzheimer’s disease (AD) and understand the mechanistic cellular and molecular pathways by which this leads to neurovascular dysfunction. Two fully-funded postdoctoral research positions are open to lead studies along these lines: - The first project will investigate the role of pericytes, the cells that control capillary diameter, in regulating cerebral blood flow in healthy physiology and in Alzheimer’s disease. This project will utilize pericyte-specific DREADDs and pharmacological/genetic manipulations to unravel the intracellular contractile machinery within pericytes, identify endogenous signals that regulate pericyte contractility, and examine how these signals may change in Alzheimer’s disease to cause neurovascular dysfunction. - The second project will investigate interactions between ischemia and Alzheimer’s disease. This project will use ex vivo (acute brain slice) and in vivo (laser speckle contrast imaging, 2-photon imaging, magnetic resonance imaging) live imaging for physiological analysis of vascular function, behavior to assess cognitive ability, and immunohistology to assess pathology. The goal is to determine how a mild ischemic episode (small stroke) alters neurovascular function over life-time and potentially synergizes with amyloid b pathology to precipitate cognitive dysfunction, and whether restoring vascular function can protect cognitive function.   The successful candidates will be responsible for experimental design, data collection and analysis, reading the pertinent literature, writing manuscripts for publication, and generally leading the project under the guidance of the PI.   The post-doc will also facilitate lab organization, participate in lab meetings, attend research seminars and journal clubs, present their work at national/international conferences, and help supervise research assistants and early graduate students in the lab.   Experience in mouse genetics, animal surgery, immunohistochemistry and western blotting, in vivo 2 photon or MRI imaging, and behavioral assessment are the main criteria. A willingness to learn and passion for scientific inquiry are a must!   Ideally, the candidate would be able to start in the lab in the fall or winter of 2023.

The current work in the CET focuses on cancer biology, chemical biology, pharmacology, toxicology and pre-clinical drug discovery/development (high throughput screening (HTS), organic/medicinal chemistry, refining functional assays), for expanded clinical use. This position will be responsible for directing a pre-clinical research team that develops and run the drug candidates, providing potentially lifesaving treatment options to patients with late stage cancers and generating primary patient derived data for research purposes. This position will also play a role in developing and executing sponsored research projects. This will include drafting SOW, providing laboratory oversight of active projects, delivering project updates, and providing guidance on future potential commercialization of research finding. Given the requirement for extensive laboratory experience, this individual will also be responsible for overseeing and providing scientific direction of key research personnel, graduate students and post-docs and their research projects. This position is also responsible for independently writing grants and providing grant support to all levels of researchers within the department and will also provide oversight and direction for manuscript writing and submission and creating statements of work for collaborative projects.
