The Foregut surgery RN Care Coordinator is a member of a multi-disciplinary team with primary responsibility for direct and indirect professional nursing care of medicine patients that have complex esophageal diseases, many of whom have oncological diagnosis.   The Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the foregut surgeons in the ambulatory / clinic setting.  Foregut surgery is a specialized field where the team nurse gets to know many of the patients closely and plays a key role in patient education and support.  The practice requires management of both benign and malignant expertise. The nurse will become familiar over time with the various treatment options and ongoing complex care.  The nurse will also perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews.  - Responsibilities include the care coordination and case management of individuals in the outpatient clinic - S/he will provide clinical support services to faculty and APP in the foregut surgery section - The nurse Coordinator will be responsible for coordinating comprehensive care for patients potentially requiring advanced procedures and will be a key contributor to ensuring that patients are guided through all stages of their treatment. - This patient population can be labor intensive given their often-oncologic diagnoses and multi-disciplinary care. This translates to increasingly complex care coordination. - The nurse coordinator will be the core of a multi-disciplinary patient care team, which includes physicians, APP, RN, MA, and PAS-R support. - The nurse coordinator will have specialized knowledge and skills in procedures, cancer care, and gastrointestinal disease management - This position serves as a coordinator for patients undergoing sequenced multidisciplinary treatment, and at times patients who travel from long distances from home - Responsibilities include but not limited to care coordination, direct patient assessment, triage, patient education, medication management, discharge and admit planning, and clinical support for the foregut clinic faculty providers. - This position will also participation in quality improvement, medical records review and  will follow existing protocols determining appropriate patient scheduling.

Registration and Accessioning - Responsible for receiving specimens and associated requisition paperwork. Assigns unique accession number; enters patient and specimen information from requisition slip into databases as required (EPIC, ULTRA/GGG, Tamtron, LabDirector/4D) for clinical service, proficiency testing and research specimens. Follows proper accessioning procedures for specimens. Initiate calls to health care providers to clarify orders, referring physicians, requesting location or patient history. Log, label and distribute specimens, requisitions, and patient files. Distribute specimens to appropriate lab or pick up area, notifying lab supervisors of special requests or circumstances. Coordinate local courier pickup and delivery from OHSU and FedEx from outreach clients. Track missing specimens and resolve discrepancies quickly with documentation and communication to lab and/or client. Ship Styrofoam specimen shippers to clients on request and document inventories. Maintain proper documentation to meet CLIA, CAP and legal standards. Follows guidelines for proper patient confidentiality per HIPPA.  - Act as registration point by gathering and/or verifying patient information including demographics, and insurance coverage. Use Lab Information System and Epic as pivot point for distribution of demographic, referral, clinical, and pathology-resulting data to multiple sources, including OHSU Medical Group and referring physicians. Transcribe from dictation or written word in to a variety of hospital and departmental applications, verifies correct ICD10 and CPT codes for billing. Reviews Dermotopathology billing workques for accuracy and make changes as necessary. Act as resource to PAS Specialists and Office Assistants in correct Dermatopathology office procedures and policies. - Process special notation, slides and other correspondence under direction of Dermpath Manager and Director of Dermatopathology and send to external customers and clients. - QC Tracking. Maintain materials control system to identify location of every block and slide in Dermatopathology custody. Maintain accurate filing system. Package and send materials and documents to referring clients and reference points.  - Order and Data Entry. Arrange and order associated clinical, diagnostic or laboratory services, coordinate authorizations when necessary. Direct patients to appropriate providers for other health care issues. Complete and route direct referrals to other clinical services.     Customer Service - Provide high quality customer service to both external customers (patient, referring providers and insurance carriers) and internal customers (OHSU health care providers and staff) that meet or exceed the service standards of the health care industry. This duty includes prompt and professional communication efforts, face-to-face customer contact skills, and crisis management, facilitate available information technology, standard complaint processing, flexible coverage of internal service needs, and the continuous application of process improvement methods and skills. All private care activities shall be conducted through the institute faculty private practice plan. This position requires undivided effort at the specific FTE. The general pattern of working hours will be set by the Director of the Dotter Institute, but deviations from general patterns may be necessary to meet position responsibilities. On average there will be 3 procedural days, 1 outpatient clinic day, and 1 academic day. Duration of this appointment and indicated salary may be changed or eliminate if a gift, grant, or contract fund supporting this position becomes unavailable. This is a fulltime appointment at 1.0 FTE. The faculty member will join and remain a member of the departmental practice plan and the faculty practice corporation for the duration of the faculty member’ OHSU appointment so long as the plan and the corporation are so approved and designated by the president. The faculty member will provide and utilize all patient care services at the University Hospital and clinics and Doernbecher Children’s Hospital unless specifically authorized otherwise in writing by the president or the president’s designee.
