Operational Leadership  - Communicates regularly with the Manager of AODs, Nursing Division Directors, Nurse Managers and professional and support services leadership for the purpose of mutual information sharing, planning and problem solving.   - Submits daily AOD report to hospital administration, summarizing hospital operations for previous 24 hours. Communicates any necessary follow-up needs with oncoming AOD. Collaborates with the HAOC and key stakeholders to coordinate an appropriate scalable response to disasters, threats, or other emergencies that could impact healthcare operations.  - Collaborates with Facilities Management to ensure timely/safe infrastructure repairs to maintain patient care operations.  - Collaborates and communicates with university stakeholders when Modified Operations may be indicated. - Screen patients for and obtain/record patient’s chief complaint, take accurate history of illnesses/surgeries (Ophthalmic and General), past ocular history, family history, drug allergies, and medications.  - Assist physician with completing/transferring patient information to the patient problem list as required per JCAHO.  - Perform ophthalmic testing as appropriate per patient.  This includes but is not limited to: - Checking/documenting distance and near vision in adults and children with and without correction - Clearly determining from patient history if the patient is needing new glasses on any given visit - Pupillary assessment to rule out afferent defect and to check pupil size (rule out asymmetry), corneal curvature, corneal pachymetry, corneal topography, color vision testing - Must be proficient in performing lensometry including reading lenses with multifocal and progressive adds, power of sphere, axis of cylinder and cylinder power, and orientation of prism and measurement of interpupillary distance.  - Perform applanation tonometry, and utilize Tonopen (noncontact tonometry) to check intraocular pressure. - Perform confrontation visual fields.  - Must be proficient in refractometry and auto refraction, fogging with astigmatic dials, cross cylinder refinement and measurement of accommodation. - Must be able to perform cover/uncover test, alternate cover test and quantify strabismus.  - Must be able to document ocular motility, presence of strabismus or nystagmus. - Must be able to perform Worth 4-dot test and Stereopsis test.  Must be able to test near point of convergence, ductions, versions and vergences.  - Must be able to perform Potential Acuity Meter testing, Glare Testing. - Instill dilating and other ophthalmic drops per physician’s instructions. - Perform calibration of automated and non-automated perimeters. - Maintain a working  knowledge of computerized visual field programs, and be able to perform Amsler Grid testing.  - Maintain a working knowledge of disease processes that may affect the visual pathway and resultant changes in the visual field due to these disease processes in order to adequately perform Goldmann (non-automated) perimetry (central and peripheral). - Perform SITA and Standard Humphrey (automated) (central and peripheral). - Assist doctors during examination as needed.  - Apply eye dressings, protective shields and dispense medications as directed by the physician.  - Provide patient instruction, education and assistance as required.  - Always be willing to assist patients in clinic areas.  - Per standard guidelines, clean exam lanes and minor surgical instruments.  - Prepare for the sterilization process, either sterilizing instruments utilizing the sterilizer in the Minor Surgery Room per guidelines or preparing instruments to be sent for sterilization.  - Responsible for room maintenance including periodic cleaning of exam lanes.  - Keep rooms stocked with appropriate instruments, as well as necessary ophthalmic drops, per physician’s direction. - Other duties:  - Take tech calls as needed to assist front desk scheduling team, refill pharmacy requests as per physician guidelines; The Director of Adult Blood & Marrow Transplantation shall lead all aspects of the BMT program including education, research, and clinical programming. The Adult BMT Director will report to the Head of the Division of Hematology and Medical Oncology. Anticipated institutional collaborative efforts for managing the BMT program will include efforts with the Doernbecher Pediatric BMT Program leader, OHSU Cellular Therapy Program Leader, Hematology & Oncology Division Medical Director, the OHSU Hematology & Oncology Division Quality Director, OHSU Vice President for Oncology Services, as well as members of other Knight Cancer Institute leadership team, to design and execute strategic initiatives. The BMT Director will serve as the Program Director for hematopoietic cellular therapy in accordance with FACT-JACIE Standards. The BMT Director may delegate specific duties to other faculty leaders in support of the BMT program. - Clinical Operations. Per FACT-JACIE Standards for Hematopoietic Cellular Therapy: The BMT Director will provide leadership for: - Administrative and clinical operations, including serving as Medical Director of the Northwest Marrow Transplant Program, coordinating care between Doernbecher Children’s Hospital and Legacy-Good Samaritan Hospital teams, and to maintain compliance with FACT standards and other regulations and policies. - Active management of the clinical program, including quality management, selection and care of recipients and donors, and cell collection and processing. Focus on achieving and maintaining national quality measures is a major focus as well as assuring Center of Excellence status with external payers. - Oversight of the medical care provided by members of the clinical program, including verification of the knowledge and skills of members. Currently, shared direction is planned for the OHSU Hematopoietic Cellular Therapy and the Immune Effector Cell therapy programs.   - Research. The BMT Director will work with the BMT team faculty, along with members of the Knight Cancer Institute clinical research management team, to ensure a broad portfolio of research, including translational research studies and clinical trials. Clinical trials will include institutional investigator initiated studies, as well as those sponsored by pharmaceutical and biotechnology companies as well as to ensure active participation in the BMT Clinical Trials Network and other cooperative groups. Ongoing commitment to maintaining institutional data base and registry will assure ongoing clinical research efforts. - BMT Clinical Trials Network - SWOG/ NCTN - Operations and Finance. The BMT Director will work with the OHSU Hospital Transplant Manager, with the Hematology & Oncology Division and Knight Cancer Institute to develop and execute strategies for focused program areas of excellence and growth and to allocate program-specific resources, including: - Faculty recruitment and team expansion - Knight strategic planning - Value Stream Management - Marketing and community outreach - CIBMTR registry - Education and Faculty Development. The BMT Director will work with other senior BMT team members, to maintain a structure for orientation and continuing education for all program members, including faculty physicians and APPS, fellows and residents, nursing, and pharmacy. The BMT Director will meet with individual faculty to monitor and support their professional development and advancement..  The BMT director will actively recruit faculty to develop the clinical and research programs and will engage in faculty building, retention and development opportunities.   - Annual faculty reviews - APP leadership meetings - Internal and external educational conferences - Assure FACT annual educational targets are BMT Program leadership roles: The following faculty leadership roles are in place or may be created. 1. Patient Identification Policy. - Labels images completely & accurately with patient demographics, position or other required documentation. - Follows through with all aspects of scanning duties in order to provide efficient and timely completion of examinations with correct data entry, begin and complete with correct billing and verification that images are in PACS. - Technologists prescreen all patients for sensitivity & contraindications to contrast medications used during the imaging exam.  - Implements the correct types of contrast and injection factors on a case-by-case basis.  - Generates a CD or DVD of imaging exams on an as-needed basis. 3.
